(if providing illustrations include several samples, not originals), note that illustrations are the responsibility of the author and/or their illustrator, full mailing address, telephone and fax numbers, an E-Mail address, a personal Biography, a pen name if used, who you are, what you do for a living and for relaxation, your aspirations, a title list of your works with a short Biography for each, two or three lines. Do not include reviews, awards or past accomplishments. Authors are judged by their work and story.
