It is based upon the author's real-life experience of conceiving a baby through in vitro fertilization. The narrator is a seven-year-old boy who tells the story of the birth of his twin sisters, Sarah and Juliet. Jackson explains that when he asked his parents for a baby brother or sister, they told him that they had to get help from a doctor at a special clinic to get pregnant. As the story develops, Jackson shares the family's emotional experiences in candid and natural language.
The author's intent is to present a realistic depiction of events that is simple enough for a young child to understand. The story also introduces the idea of adoption as an option, but in the end the family's fourth and final attempt at in vitro fertilization is successful. The excitement of the pregnancy is shared by Jackson's parents and his Grandmother as they discover that they are expecting twin girls in their family. A year after the birth of the twins, Jackson reflects that, "Sometimes it's not easy being a big brother to my super special sisters, but most of the time it*s just great!"
